Share your experience of leaving hospital

Have you recently left hospital, or supported someone who has? Healthwatch North Northamptonshire wants to hear about your experience – what went well, what could have been better, and whether the right information, care and support were in place when you left hospital.

Healthwatch North Northamptonshire is working with Healthwatch England on a national project to better understand people's experiences of leaving hospital.

From 17 August to 9 October 2026, we are inviting people across North Northamptonshire to share their experiences through the Hospital Discharge Survey.

We are particularly interested in hearing from people whose experience of leaving hospital was delayed or difficult. We also want to hear about what worked well and what could have made the experience better.

Who do we want to hear from?

The survey is for people who have experienced hospital discharge, as well as people who supported someone through the process.

We would like to hear from:

  • people who have been discharged from an NHS hospital in England in the last 12 months;
  • carers who supported someone leaving hospital;
  • family members who supported someone; and
  • friends who helped or supported someone through their discharge.

We are particularly keen to hear from people from Black/Black British and Asian/Asian British communities across North Northamptonshire, helping to ensure these experiences are represented as part of this national work.

The participant information sheet confirms that the survey is for people discharged from an NHS hospital in England in the last 12 months, or carers, relatives and friends answering about someone they support. 

What do we want to know?

We are particularly interested in hearing from people who had a delayed or difficult experience of leaving hospital, or from someone who supported them.

This might include situations where:

  • your discharge was delayed by at least a day;
  • you left hospital before you felt ready;
  • the care, support, equipment, medication, transport or follow-up you needed was not in place; or
  • the information you were given was unclear.

We also want to hear about what went well during your experience.

By sharing your experience, you can help us understand how delays and difficulties affect people, whether people are leaving hospital safely and with the right support, and what could be improved.

The survey opens at 8:00am on Monday 17 August 2026 and closes on Friday 9 October 2026.

It should take around 10–15 minutes to complete, and you do not have to answer any question you do not want to.

Need help completing the survey?

We want as many people as possible to have the opportunity to take part.

Paper copies of the survey are available, and Healthwatch North Northamptonshire staff and volunteers can support people who may need help completing it.

Help us reach more people

If you are part of a community group, faith group, support group or voluntary organisation, we would love your help reaching people who may be eligible to take part.

If your group is meeting or holding an event during the survey period, we may be able to arrange for a member of our team or one of our project volunteers to visit your group, explain the project and support eligible people who would like to participate.
