Prescription Prepayment Certificates (PPCs): saving money on NHS prescriptions

Prescription Prepayment Certificates (PPCs) are an NHS scheme designed to help people in England reduce the cost of prescription charges. They are particularly useful for people who need regular medication, as they allow unlimited NHS prescriptions for a fixed price over a set period.

For many people, a PPC can offer significant savings compared to paying for each prescription item individually.

What a Prescription Prepayment Certificate is

A PPC is essentially a “season ticket” for NHS prescriptions. Instead of paying per item, you pay a fixed upfront cost or by instalments (through direct debit) which covers all NHS prescription charges during the validity period.

It covers:

  • All NHS prescription items (not private prescriptions)
  • As many items as needed during the certificate period
  • Prescriptions from any NHS pharmacy in England

It does not cover other healthcare costs such as dental treatment, wigs, or medical appliances.

How PPCs work

When you have a valid PPC, you simply show your certificate details when collecting prescriptions. The pharmacy will check your exemption status, meaning you do not pay per item.

There are two standard options:

  • 3-month PPC
  • 12-month PPC

Once purchased, your cover starts from an agreed date and lasts for the full period, regardless of how many prescriptions you need.

How much a PPC costs (and what you could save)

As of current NHS charges:

  • A single prescription item costs £9.90
  • A 3-month PPC costs £32.05
  • A 12-month PPC costs £114.50

A PPC usually becomes cost-effective if you regularly need prescriptions:

  • 3-month PPC: typically worthwhile if you need 4 or more prescription items in 3 months
  • 12-month PPC: usually worthwhile if you need 12 or more items in a year

Depending on usage, people can save over £100 per year, and in some cases significantly more for those on regular medication.

Who might benefit most

A PPC may be particularly helpful for:

  • People with long-term health conditions requiring regular prescriptions
  • Individuals undergoing treatment involving multiple medicines
  • People collecting repeat prescriptions each month

Before purchasing, it is also important to check whether you already qualify for free NHS prescriptions, for example due to age, pregnancy, certain medical conditions, or low-income support.

How to apply for a PPC

Applying is straightforward and can be done in several ways:

Online (fastest option):

  • Apply via the NHS Business Services Authority website

By phone:

  • Call the PPC order line on 0300 330 1341

At a pharmacy:

  • Some pharmacies can help you apply in person

You will receive either:

  • A digital certificate by email, or
  • A paper certificate by post

Payment can be made upfront, and for 12-month PPCs, spreading the cost via monthly Direct Debit may also be available.

What to do next

If you regularly pay for NHS prescriptions, it may be worth reviewing whether a PPC could reduce your costs. However, it is always advisable to:

  • Check eligibility for free prescriptions first
  • Consider how many prescription items you typically use
  • Compare costs before applying

PPCs remain a simple and effective way to support people with ongoing medication needs and help reduce the financial burden of healthcare costs.
